MEDICAL CONDITION VALIDATION

Second-year and older cadets were required to validate their medical condition before 31 December 2011 by completing the Annual Basic Health Questionnaire DND 2571 and/or the Detailed Health Questionnaire DND 2570. Effective immediately, if the following cadets do not bring their forms in to the Admin Office, it will no longer be possible for them to participate in any special activities, nor apply for summer camps. Therefore, it is of primary importance for any cadet whose medical condition has not been validated, to have it done as soon as possible.
